The Federal Deposit Insurance Corporation (FDIC) website generates "Failure and Assistance Transaction Reports" of bank failures back to 1934 based on user-selected criteria.

The FDIC website also has a listing of bank failures back to 2001

The Annual Report of the Federal Deposit Insurance Corporation (FDIC) has information about failed banks and is available in print back to 1981 in Firestone stacks, call number HG1662.F33.  (1996+ online)

The Bank Regulatory module within WRDS also has information on bank failures/mergers since 1986.
